What is Foundations Direct Care?

Foundations DPC is the solo family practice of Dr. Emily McLaurin. Foundations Direct Care provides hope and healing by empowering individuals and families to make healthy decisions so they can thrive in their communities. We provide peace of mind and convenience through unhurried visits, price transparency, shared decision making, and a commitment to care for the whole person. We know that as your health improves today, lives will be changed for generations to come.

Foundations DPC currently doesn’t accept insurance because we believe the only people involved in your healthcare should be you and your physician. Learn more about us.

Due to the nature of the clinic, we are not a walk-in clinic. We are by appointment only, though appointments could be minutes after requesting it.

Is this Concierge Medicine?

Not exactly, but yes. While the two types of models are similar, they are not synonymous. Generally, in a concierge practice, the patient typically pays a higher retainer fee plus other health insurance plan obligations (e.g., co-pays, out-of-pocket expenses) and the practice continues to bill the patient's insurance carrier. By eliminating the staff needed to bill insurance, we've decreased the fee and offer the same services including access to your physician's cell phone, same/next-day extended visits and included procedures.

What are the benefits of joining Foundations?

Whether you have amazing, mediocre or no health insurance, Foundations revolutionary membership model helps achieve financial control over your healthcare as 85-90% of typical healthcare needs can be taken care of by a primary care physician.  When you need to see your doctor, you won't need to wait 2 weeks for the next open appointment or an hour in the waiting room. We offer timely same/next-day appointments — no more waiting! And on top of that, we offer complete telemedicine availability (text, email, videocalls, phone).  Whether it's a quick question or you're just out of town, often the problem can be solved without being seen. If you want your loved ones involved in your care, they can be! Need longer visits to go over complex problems? Done! Are you tired of feeling like you blinked and your doctor was out of the room? Not anymore. When you do need to see the doctor, you won't be rushed or feel like you're coming back "so they can bill you again." We can get you in and out quick, or have hour long visits — all depending on your needs. Your care is tailored to you and your needs.

And to top it off, all your procedures done in office are already included in your membership fee. Labs and imaging are offered at heavily discounted prices or you can use your HSA/flex/insurance. We also have substantially discounted prices on medications.  And as our practice grows, so will in-house benefits, including yoga therapy to nutrition counseling to counseling services. But the best part, in our opinion, is having a physician who cares for you like she'd care for her own family. Save money, save time, and receive outstanding healthcare.

Why monthly fees?

In the current healthcare system, doctors are incentivized to see as many patients as possible during the day. After all, it is a business. Your doctor is reimbursed by your insurance company (at an undisclosed negotiated rate) when they see you - and only when they see you. Often, the doctor does not even know these negotiated rates off the top of their head.  Adding a strep test, depression screen or review of lab work increases the cost of the visit — and because of deductibles and out-of-pocket costs, patients often get bills later.

Having a low monthly membership fee, allows us to operate the practice on a day-to-day basis and build-in costs  so you don't receive unplanned bills. Our goal is to be transparent with our pricing and services, all while offering our patients the best-in-class healthcare previously only dreamed of: extended appointments, full access to your physician (not a nurse help line), relaxing clinic atmosphere and a partner in your healthcare decisions.

How many patients do you care for?

Our enrollment is limited to 500 patients. A traditional primary care clinic maintains approximately 3,000 patients and an urgent care typically see more than 600 in a week! By keeping our total member enrollment low, we can continue to offer unparalleled access and individualized care.

Do you take Medicare patients?
We do. However, we do not bill Medicare for our services. You will pay a membership fee just like all members. As a Medicare patient, you can use your Medicare or supplement plan for labs, imaging, medications or specialist referrals outside of Foundations.
What if I need labs or imaging done?

We have negotiated wholesale prices with local labs.  Please reference the Pricing Sheet to get an idea of our negotiated rates.

You can also use your insurance to pay for labs and imaging. Think about your health insurance like car insurance. You wouldn't submit a tiny fender bender to your insurance if your deductible wasn't met, why submit small lab expenses?

I have a lot of health problems, do you charge extra?

Our monthly membership fees are the same price for every patient, regardless of pre-existing conditions. Some labs, tests, and procedures may cost extra but we will negotiate wholesale pricing on these!  We will manage your health problems to the extent of our practice limitations and will refer to a specialist when necessary.  And if a referral is needed, we will still make sure everyone will be in the loop.

What if I‘m healthy and never see the doctor?

This is a great problem to have! But shouldn't your doctor also focus on helping you prevent disease and maintain wellness? When was the last time you had an in-depth preventative wellness visit? As we grow, we hope to continue to offer more services including nutrition counseling, wellness workshops, functional medicine, etc. But in the meantime, don't let that pesky sinus infection keep you down. Whether you're traveling or you're home here in Oklahoma City, when you have an acute problem we're here for you! And best part is — no wait times and same-day/next-day visits! Getting ready to travel internationally? We can use telemedicine to help solve most medical problems while you're abroad. Never again do you have to worry about finding an urgent care or wondering about a provider's skill level. Think of our retainer as a way to help cap your medical expenses.

Can I use my HSA or FSA to pay for your services?

Consult your tax or financial advisor. There is no distinct answer currently. It's very inconsistent and confusing, but there is current federal legislation sitting in a committee that will clarify the issue! We will update this/notify patients as soon as legal clarification is made.

What if I need to see a specialist?

We can make referrals like any traditional primary care doctor (unless you have a HMO, this can sometimes make it tricky because they require a "gatekeeper", but we will work with you). But, here at Foundations, not only do we have the ability to have longer visits to truly discuss your health issue with the goal of minimizing referrals, your membership includes e-consults with specialists through RubiconMD in an effort to decrease the need for formal referrals. This connects your doctor with a specialist in 4 hours on average, way better than that traditional 4 week wait!

What if I need to be admitted to the hospital?

If you feel you have a life-threatening emergency, call 911 immediately.

Foundations does not have admitting privileges at any hospital. If you need to be hospitalized, we will work to help coordinate care during and after your hospitalization. While no one can predict when your appendix will rupture, the goal of our clinic is to minimize hospitalizations by focusing on preventative care and increasing accessibility to your doctor.

Remember, you will still need insurance to cover any hospitalization.

What if I need to go to the emergency room?

If you feel you have a life-threatening emergency, call 911 immediately.

While we will focus on preventative care, some things may require evaluation at an emergency room as they would at any clinic.

What if I don't have health insurance?

No worries, you can still be a patient here at Foundations. We offer all imaging and labs at substantially discounted prices! This is regardless of your insurance status! We do recommend insurance though — but this is for the rare big things like surgeries, true emergencies, chronic diseases such as cancer. Think of it like car insurance — we maintain it because it's the law and to help us pay for any major problems. We don't use car insurance to cover your car's routine maintenance or small problems, so why should we use health insurance like that? There's a reason our premiums keep rising. Because all of your primary/urgent care needs are met with our low monthly membership fee — we recommend a high deductible plan.

What if I already have health insurance?

Perfect! As stated above, we believe it is important to have health insurance to help cover the big expenses. If you're worried about "paying double," don't forget you already do! You pay once with your premium and then again with your deductible and out-of-pocket. At Foundations, we will work to save you money by minimizing total out-of-pocket expenses with our predictable monthly fee and our value-rich clinic! Or better yet, change to a high deductible plan as this will most likely bring you the best savings when pairing it with our clinic. We can guide you to the right avenues to find appropriate plans.

Do you work with small businesses?

Absolutely. We'd love to talk to your business about helping offer an affordable and ideal healthcare solution to its employees!

How do I pay?

We accept all major credit cards and direct bank transfers (ACH). Please call with any other questions.

What happens when the doctor goes out of town?

Dr. McLaurin is always be available via telemedicine and patients will be notified of any such events ahead of time. This is one of the big differences with Foundations compared to a traditional practice — you will ALWAYS have access to your doctor. NOT just a nurse answering service. In the event you need to physically be seen while Dr. McLaurin is unavailable, she will partner with trusted area physicians to ensure her patients receive quality care.

Can I reach the doctor after hours?

Of course! This clinic was created so you can call your doctor when you need them. Twist your ankle playing basketball and don't know what to do? Now you know who to call! Cut your finger while cooking dinner? Text a pic and we'll discuss what we need to do. Who hasn't come down to their last pill and realized they were out of refills? There are countless examples of after-hours needs. That's the beauty of Foundations DPC and having a real relationship with your doctor!

What do you NOT do?

We do not treat chronic pain or carry narcotics in our office. We also do not provide prenatal care (obstetrics). See above question on Medicare. At this time, a limited supply of vaccinations are available, and we can discuss availability when you visit.

What are the office hours?

Monday through Friday we maintain general hours and we can arrange for visits outside of these hours if appropriate. However, we are always available via tele-medicine (see below).

Are there other types of “visits”?

If the doctor deems you don't need to come in, let's avoid the hassle. Many questions can be answered without an in-person visit, so we offer texting, email, phone calls, and video-conferencing to our members at no additional fee. We also offer home and work-site visits for an additional fee.

Just Say No' to the Medication Switches

As the start of the new year has come and gone, many of my previous patients have been inundated with the ritual beginning of the year change of prescription requests.  Often the story goes like this:

  • "The Lantus (glargine insulin) that you prescribed for Mrs. Jones is no longer covered by her insurance; please change it to Basaglar (glargine insulin), which is covered and the new preferred formulation."
  • "Patient's Xarelto, a direct oral anticoagulant, is no longer being covered by their insurance plan this year; please change them over to warfarin."
  • "Your prescription for Mr. Smith's Valsartan is no longer valid; it was rejected when he attempted to fill it at the pharmacy. Please change to an alternate covered therapy."
  • "The mesalamine that your patient has been using for 20 years now has a $600 co-pay; please change her medication to something -- anything -- else."

All very nicely worded, all very polite, and all completely ridiculous. This process highlights just one more example of how we have let ourselves be railroaded into being told how to take care of our patients by the insurance companies.  

The backstory to all of this medical switich is that insurance companies are in this to make money, and that every year they renegotiate with the pharmaceutical companies, and save a nickel on each prescription when they change from Brand X over to Brand Y as their "preferred medication" in a particular class.

One of the patients this year for whom I had to change their insulin formulation, was changed from that very formulation to a different one last year, and that had been changed again the year before.

This cannot help us take care of patients; this cannot lead to better care. This can only lead to more profit for insurance companies and pharmaceutical manufacturers. 

We also hear from the insurance companies, "Your patient is entitled to 30 visits with their physical therapist this year; please enter a new referral so that this can be processed by their insurance company." For whose benefit?

So much of this leads to the beginning-of-the-year bombardment that we all face: the onslaught of messages that really don't help us take care of patients, that really don't lead to better care, more collaboration, or improved communication. It is a shame on us that we have let this happen, that we continue to let this happen. This is the busywork that grinds us down, that bruises our souls, and it's time we said no. Just, no.

As a member of the Foundations DPC community, you will not have to face these challenges batting Goliath - the insurance companies.  Your membership, we will work with you to ensure that you are getting the best and most appropriate treatment for your specific needs - this will not be dictated based on which insurance company you are with (or even which side of the bed the insurance company representative woke up on).  At Foundations DPC, you are our primary focus and you are in good hands with us.

Happy new year! I hope the holidays have been full of rest and sweet times with friends and family. Looking back at 2022, I want to thank all of you for entrusting my team and me to care for you. I can’t believe our doors have been open for over a year now, and I’m eager to see what lies ahead in 2023.

One Conscious Breath. A message from Denean Knapp​

Breathe. How often we hurry through life and its unending fast-paced “hustle” without stopping to ponder how it’s possible to fling our bodies at record-breaking speeds through space. Many systems are required to “symphonically” integrate to give us life to live our American lifestyle. This month let us focus on THE RESPIRATORY SYSTEM.

What a precious gift, the gift of breath. Can you acknowledge the gift of “conscious breath” in our distracted world? I challenge you this quarter to swap the interruption of “scrolling” with the interruption of ONE CONSCIOUS BREATH. Just one. Try it. Then notice.  My prayer for you this quarter is that you would be able to stop once a day and take a deep breath - then notice the miraculousness of ONE CONSCIOUS BREATH.

Benefits of Direct Primary Care Model

Direct primary care (DPC) is a healthcare model that is gaining popularity as a way to provide more affordable and accessible medical care for patients. DPC is a subscription-based model in which patients pay a monthly or annual fee to their primary care physician for unlimited access to medical services.

One of the biggest benefits of DPC is the cost savings for patients. Traditional primary care models rely on insurance and can be expensive, with high copays, deductibles, and out-of-pocket costs. In contrast, DPC eliminates the need for insurance, and the monthly or annual fee is often significantly less than the cost of insurance premiums. For example, a DPC practice might charge $50 to $150 per month, while the average monthly premium for an individual insurance plan is $440 per month.

Another benefit of DPC is the increased access to medical care. Patients can typically schedule appointments within a day or two, and they have direct and unrestricted access to their primary care physician. What are Platelet-rich Plasma Injections and why get one?

Ultrasound-guided platelet-rich plasma (PRP) injections are a cutting-edge medical treatment that is increasingly being used to treat a wide range of conditions. PRP is a concentrated form of the patient's own blood that contains a high number of platelets and growth factors. These growth factors promote healing, and when injected into an area of injury or damage, they can accelerate the healing process.

One of the most well-established uses for ultrasound-guided PRP injections is in the treatment of chronic tendinopathies. Tendinopathies are common conditions that result from overuse or degeneration of tendons and can cause pain and disability. A systematic review of the literature published in the Journal of Orthopaedic Surgery and Research in 2019 found that ultrasound-guided PRP injections were more effective than other treatments, such as physical therapy or corticosteroid injections, in improving pain and function in patients with chronic tendinopathies.

Another area in which PRP injections have been found to be beneficial is in the treatment of osteoarthritis. Osteoarthritis is a degenerative joint disease that affects millions of people worldwide. According to data from the Kaiser Family Foundation, the average annual out-of-pocket cost for an individual with employer-based health insurance was $1,218 in 2022. However, for those without insurance, the average annual out-of-pocket cost can be significantly higher.

How Foundations takes care of diabetic patients

At Foundations, we provide a 360 approach in meeting the individual needs of our members with diabetes (type 1 and type 2).

  1. Regular monitoring of blood glucose levels: Patients with type 1 diabetes need to regularly check their blood sugar levels in order to adjust their insulin doses and monitor for potential complications.

  2. Insulin therapy: Patients with type 1 diabetes require insulin therapy to replace the insulin that their bodies are not producing. Our staff works with you to determine the appropriate insulin regimen, including the type and dosage of insulin.

  3. Diet and exercise: At Foundations, we provide guidance and support for the patient to maintain a healthy diet and engage in regular physical activity to help manage blood sugar levels.  We include wellness consults and build in a holistic therapies to complement the management of your insulin.  

  4. Regular check-ups: The patient should have regular check-ups at Foundations to monitor for complications and adjust their treatment plan as needed.

  5. Education and self-management: We have a library of education and resources to help you learn how to manage your diabetes, including how to recognize and treat high and low blood sugar, how to administer insulin, and how to recognize and prevent complications.

  6. Communication with the Doctor: Through Foundations, you have an open line of communication with Dr. Emily McLaurin, who can provide guidance and support on complex cases, and also refer patients when needed.

Bioidentical hormones replacement therapy (BHRT) with pellets

Bioidentical hormones are processed hormones designed to mimic the hormones made by your body’s glands. Taking bioidentical hormones can help people who experience symptoms of low or unbalanced hormones. This is often the case for people experiencing symptoms of perimenopause or menopause.

Hormones are chemicals made by your endocrine glands. They are messengers that tell other parts of your body how and when to work. Hormones affect many systems and functions in your body. Even the slightest imbalance can cause symptoms that interfere with your day. Healthcare providers may recommend hormone replacement therapy as a treatment for these symptoms.

Bioidentical hormone therapy (BHRT) uses processed hormones that come from plants. Estrogen, progesterone and testosterone are the most commonly used bioidentical hormones.

Some prescription forms of bioidentical hormones are premade by drug companies. The U.S. Food and Drug Administration (FDA) has approved certain types of bioidentical hormones. Other forms of bioidentical hormones are custom-made by a pharmacist based on a healthcare provider's prescription. These are compounded (or mixed) bioidentical hormones.

May Melanoma Month: Protect Your Skin and Stay Sun-Savvy This Summer

May is Melanoma Awareness Month, and as your Direct Primary Care physician, I want to help you stay sun-savvy and protect your skin from the harmful effects of ultraviolet (UV) radiation. Melanoma, the most dangerous form of skin cancer, can develop from excessive exposure to the sun or tanning beds. However, with the right precautions and early detection, it is often preventable and treatable.

The first step in protecting your skin is understanding the importance of sunscreen. A broad-spectrum sunscreen with an SPF of at least 30 is essential to shield your skin from both UVA and UVB rays. Remember to apply sunscreen generously at least 30 minutes before sun exposure and reapply every two hours or after swimming or sweating.

In addition to using sunscreen, other sun protection measures include wearing protective clothing, seeking shade during peak sun hours (usually between 10 AM and 4 PM), and wearing sunglasses with UV protection.

It's also crucial to monitor your skin for any changes or new growths. Regular self-examinations can help detect melanoma early when it's most treatable. Be sure to check your skin once a month, paying close attention to any moles or spots that exhibit the ABCDEs of melanoma: Asymmetry, Border irregularity, Color variations, Diameter larger than a pencil eraser, and Evolution or change in appearance.
